Children at High Risk Should Be Screened for Elevated Cholesterol

Children with a family history of dyslipidemia or premature cardiovascular disease should be screened for high cholesterol starting as early as age 2 and no later than age 10, according to an American Academy of Pediatrics report published in Pediatrics.

Replacing its 1998 policy statement, the academy says that a child should also be screened with a fasting lipid profile if the family history is unknown or the child has other cardiovascular risk factors (e.g., BMI at or above the 85th percentile, blood pressure at or above the 95th percentile, cigarette smoking, or diabetes).

Dietary changes and increased physical activity are recommended for overweight pediatric patients who have high triglycerides or low HDL concentrations, but pharmacologic intervention should be considered in patients at least 8 years old who have high LDL levels.

Physicians should aim to get children's LDL concentrations below 160 mg/dL, but in high-risk children, target concentrations of 110 mg/dL may be warranted.
